Implantology – Fixed teeth
At the Cabinet Dentaire de Nyon, we are specialized in oral surgery.
Our approach has always been to reduce the invasiveness of procedures and to improve patient comfort. With over 20 years of experience, we make sure that you no longer have to worry about dental procedures and that you will find the smile you have always dreamed of in our office.
Fixed teeth for better health and greater comfort on a daily basis
Oral implantology – or dental implantology – makes it possible to reconstitute lost dental elements by inserting an artificial root called an “implant” into the jawbone.
Fixed dental prostheses (crowns, bridges) or removable dental prostheses, commonly called “dentures” (stabilization of complete or partial removable prostheses) can be attached to this artificial root.
Whether a single tooth or a complete jaw is to be reconstructed, modern implantology guarantees solutions for every situation, ensuring impeccable aesthetics and function.
Fixed teeth in one day (immediate loading procedure)
Traditional implantology vs. immediate loading implantology.
In the traditional procedure, which is still followed by most dentists, the implants are placed first and then a period of several months must pass before the final prosthesis can be installed.
Traditional implantology vs. immediate loading implantology.
In the traditional procedure, which is still followed by most dentists, the implants are placed first and then a period of several months must pass before the final prosthesis can be installed.
Indeed, in traditional implantology, several operations are often necessary (dental extractions, bone reconstruction, reopening of implants) and the treatment lasts from 6 to 12 months during which one must wear dentures, sometimes ill-fitting because of the operated tissues, which affect the masticatory function, ultimately leading to many restrictions in daily life.
In addition, this procedure has a very high cost that is far from being within the reach of everyone.
But today, thanks to new knowledge and technological advances, it is fortunately possible to perform in a single session the placement of implants and prostheses that are attached to them (the treatment is done under sedation with the greatest comfort)
This allows almost normal mastication at the end of the operation (immediate loading).
Moreover, by using teeth entirely made of ceramic or composite, the aesthetics are also perfectly restored; not to mention that you will finally be able to enjoy everything you want!

Computer-assisted implantology
Virtual planning allows the computer to preview the ideal position of the implants and teeth on the three-dimensional bone model.
Once the position of the implants has been established, a laser-printed guide will allow the surgeon to reproduce exactly the same position that was planned virtually during the operation.
Computer-assisted implantology is an up-to-date, comfortable and reliable treatment: a revolution in modern implantology.
The surgical guide allows the drill to be precisely guided to reproduce the implant position decided during virtual planning.
With this minimally invasive method, there is no need for a surgical incision as only a tiny hole is made through the gum to place the implant.

Four solid pillars for a bright smile: the All-On-4 technique
The All-On-4 technique (meaning “all [fixed teeth] on 4 [implants]”) offers the best solution for replacing teeth in a minimum of time: the temporary prosthesis is screwed onto the four implants only a few hours after the end of the operation and without a bone grafting procedure.
The name of this technique comes from the number of implants inserted, which is usually four (All-On-4). At least four artificial roots are needed in each jaw to support a complete dental arch.
Depending on the situation, up to six implants (All-On-6) may be placed in the patient’s upper and/or lower jaw.
The implants are inserted in strategic positions that are precisely calculated and ideally spaced.
The two posterior implants can be positioned at an angle of up to 45°.
With the All-On-4 method, we have a tested and scientifically proven clinical program that has already helped many people.
The All-On-4 technique provides an extremely solid geometry on which a very stable bridge can be anchored within a few hours, while avoiding the time-consuming process of a jawbone augmentation procedure.
That’s why we call it “fixed teeth in one day”!
(immediate loading method).
Today, thanks to the latest knowledge and techniques, new teeth can be placed in record time and at a very affordable price.
With the All-On-4 procedure, all the care is done at once, with only four to six implants per jaw. As a rule, only one surgical procedure is required.
Thanks to the special inclination of the implants, bone grafting is usually not necessary and it is possible to use longer, particularly stable implants that allow immediate loading of the fixed prosthesis.
A few hours after the implants are placed, a temporary bridge designed by our dental laboratory is screwed in place and the patient can leave our office with fixed teeth ready to be put to the test.
The final bridge is made within 4 to 12 months.

Fixed teeth in one day
- Faster and easier bone integration of implants
The immediate loading procedure can be implemented if the following conditions are met:
- Availability of sufficient bone
- Immediate implant stability
The method can be used for a single tooth, for a group of teeth or for the entire jaw (All-On-4 technique).
Numerous scientific studies have proven that the success rate of implants placed immediately is even higher than that of implants placed using traditional methods, provided that the planning and operation are carried out carefully by an experienced professional.
What does it mean for you?
- You come to our clinic in the morning with a denture or teeth in a very bad condition, and you leave by the afternoon with a new dazzling smile!
- A single surgical session is required to remove the diseased teeth, place the implants and set the fixed teeth.

Immediate loading
(fixed teeth in one day)
Immediate loading is an advanced technique in modern implantology that allows you to place a temporary prosthesis on a freshly placed implant.
This treatment gives you the advantage of being able to leave the office with fixed teeth (a bridge) on the same day that the implants are placed, and to have your smile back in just one day.
In order for a dental implant (artificial root) to be stable and to support the tooth, certain conditions must be met. Modern implantology has developed with the evolution of biological knowledge about the bone healing process.
Thus, by strictly respecting scientifically validated protocols, it is now possible to install the tooth on its artificial root the same day and allow you to return home with one or more fixed teeth.
To better understand what the “revolution” of immediate loading represents, it is necessary to recall the “classic” implant protocol, developed more than 35 years ago, which was as follows
• intervention for the extraction of the tooth
– 3 to 4 months of waiting
– intervention for the insertion of the implant (the implant was left to heal hidden under the gum)
– 4 to 6 months waiting time
– implant exposure procedure (the implant was previously hidden under the gum)
– 1 month of waiting
– taking the impression
– 1 week waiting time
– placement of the tooth/bridge
This means a total treatment period of 9 to 12 months, with three surgeries and several control sessions.
However, in 60 years of research, biological knowledge and technological advances have made it possible to simplify the original protocol and to develop much more effective and, above all, less invasive protocols:
– a single procedure for the extraction of teeth, immediate insertion of implants and immediate placement of temporary teeth
– 4 to 6 months of waiting (with the temporary teeth in place) before the realization of the definitive teeth (more beautiful and durable than the temporary teeth)
A total of 4 to 6 months of treatment, with only one surgery. In other words, there is no wait between the loss of teeth and the installation of new fixed teeth.

Thanks to 30 years of research and professional intuition, a Portuguese doctor, Professor Paulo Malo, has developed the All-On-4 technique, which circumvents the anatomical “obstacles” mentioned above. Thus, the posterior implants are placed at an angle of 30-45°, which allows the use of longer implants, high stability and avoidance of contact with the maxillary sinus and the mandibular nerve.

Due to missing teeth and/or infections, there is often insufficient bone available for implant placement.
Due to anatomically delicate formations (e.g. maxillary sinus, mandibular nerve), it is often not possible to use the traditional implant technique without bone augmentation

This method has important benefits for you:
- Faster healing thanks to a single surgery (one-step technique)
- Immediate placement of fixed teeth allowing for an immediate return to normal life
- Cost reduction
How does the All-On-4 treatment work?
To give you an idea of how an All-On-4 treatment works, we have summarized the typical steps in the following timeline. Of course, depending on the specificity of individual situations, we will adapt the treatment accordingly.
1 month before the operation
After the initial consultation, analysis of X-rays and optical scan images, we invite the patient to come in and discuss the planned procedure in detail.
1 to 2 weeks after the operation
Lors de la séance de contrôle, le médecin retire les fils eQt vérifie la prothèse.
4 to 6 months after the operation
The day of the intervention
We will welcome you to the Cabinet Dentaire de Nyon.
Before the operation, you will have the opportunity to speak with the surgeon to ask any open questions.
A pre-medication with compresses will be administered to take away the stress of the moment

Safety
The team at Cabinet Dentaire de Nyon has extensive experience in implantology. Thanks to the All-On-4 technique, we have helped many patients obtain a fixed prosthesis in a single day.
This professional and surgical experience is crucial to prevent or minimize complications in complex cases and allows us to greatly reduce the rate of implant loss. Our patients’ satisfaction is ultimately the best reward and a source of great pride for us.
Comfort: conscious sedation
At the Cabinet Dentaire de Nyon, your comfort and safety are important to us. That is why we take care to alleviate the stress and discomfort that may be associated with treatment, in particular by using sedation.
Conscious sedation (from the Latin sedare, which means to soothe, to calm) is a “semi” anesthesia that puts the nervous system to sleep slightly, giving the patient a feeling of calmness, but without making him or her lose consciousness.
This is why we systematically perform all long or complex procedures under “conscious sedation” by intravenous route.
Everyone can benefit from conscious sedation, which has several advantages
– relaxation during the procedure
– reduction of the intervention time
– reduction of physiological reactions to surgical stress
At the end of the procedure, the patient hardly remembers the operation and the time has passed without him/her noticing it.
Sedation provides more than comfort: it ensures your safety.
Cost of All-On-4 treatment
It is important for the patient to clearly understand the cost of the All-On-4 method and what it includes.
That’s why, in the interest of transparency, after careful evaluation of the data collected, we create a treatment plan and a firm quote.
To give you an idea, the cost of restoring a jaw with the immediate loading method ranges from CHF 20,000 to CHF 38,000, depending on the complexity of the case, the prosthetic solution and the type of sedation chosen (conscious sedation administered by your surgeon or general anesthesia involving the assistance of the anesthesia team).

F.A.Q. Fixed teeth in one day
F.A.Q. Computer assisted implantology
F.A.Q. Implantology – Fixed teeth
F.A.Q. Computer assisted implantology
How does computer-assisted implantology work?
Here are the steps of a computer-assisted implant treatment
– The dentist or assistant makes a three-dimensional X-ray examination or CBCT (Cone Beam Computed Tomography) and an optical impression of the patient’s mouth.
– A specific software program processes the data obtained.
– The surgeon can virtually simulate the operation that he will perform on the patient by placing the implants in the most correct position on the computer screen.
– Based on the preoperative programming, the laboratory prints a 3D surgical guide made to measure.
– The surgeon inserts the implants into the patient’s mouth in the exact position for which they were designed.
– Immediately after the implant placement procedure, the dentist can install the temporary teeth, as the fully customized prosthesis could be made before the surgery.
Computer-assisted implantology can be used with both the flapless technique and conventional open surgery to increase precision, reduce surgery time and increase patient safety and comfort.
Is computer-assisted surgery painful?
No.
The procedure is done under local anesthesia like any oral surgery. The advantage is that all the time spent programming on the computer is saved during the surgery. More precision and less surgery time means less post-operative sequelae and more comfort for you.
Can all doctors do computer-assisted implantology?
In order to “simplify” the rather complicated implant technique with the help of digital programming, it is necessary to have a long experience of traditional surgery. Indeed, when you are in front of the computer screen to program the surgery, you must be able to display all the possible variables that may exist in “real life” and prevent them. In case of complications (very rare), one must be able to intervene instantly thanks to the techniques and knowledge of traditional surgery.
Can computer-assisted implantology still be performed?
The help of technology is invaluable, especially in the diagnostic phase, and performing a surgical guide certainly promotes the precision and speed of the surgery.
Therefore, it is always possible to benefit from computer-assisted planning when deciding when to perform a surgical guide (“guided” implantology proper).
F.A.Q. Fixed teeth in one day
For what type of patients is the "one-day fixed teeth" method suitable?
This method is perfect for anyone who does not want to be without teeth, even for a short period of time.
The most frequent cases are :
1. You have to remove a single tooth (accident, decay, infection) and you don’t want to walk around with a hole in your mouth while waiting for the traditional implant procedure (6 to 12 months).
2. You have to extract all your teeth (periodontal disease, loosening of teeth, gradual deterioration of the dentition), but you want to be able to eat, speak and smile without the constraints of a complete prosthesis (denture).
3. You already have no teeth and you wear dentures that annoy you because they move, snap and deprive you of the taste of food.
Someone said that you have to wait 4 to 6 months to put a tooth on an implant when you put them right away: why?
Our answer: experience and competence!
Discoveries in biology are progressing and the design of implants is evolving. With a good knowledge of the healing mechanism, the use of implants with shapes adapted to this type of procedure, and adequate technical training, we can obtain results that were unthinkable just a few years ago.
Are there additional risks associated with immediate loading?
No. International scientific publications have amply proven that this system actually presents lower risks than the traditional protocol, provided of course that immediate loading is performed by an experienced surgeon.
Why do few physicians implement the immediate loading procedure?
You should ask them! This may be because it takes a long time to master this technique.
However, there is no reason not to use immediate loading if the conditions are right.
F.A.Q. Implantology – Fixed teeth
How to get rid of dentures?
Although a part of many people’s daily lives, removable dentures are often a source of much discomfort. It can lead to social isolation because people are afraid to smile or appear in public, fearing that dentures will move or clash in an embarrassing way.
Removable dentures can also indirectly affect overall health, as people with removable dentures end up eating poorly due to their inability to chew, rather than eating well with a healthy, balanced diet.
The quality of life is severely affected because the removable prosthesis that covers the palate prevents the perception of the taste of food.
Moreover, in the absence of the internal stimulation usually exerted by the roots of the natural teeth and under the effect of unequal chewing, the jaws end up resorbing. As a result, the lips, chin and cheeks sag and the person ends up looking older and sicker than he or she actually is.
How can a dental implant improve your life?
Until now, when all teeth have fallen out or had to be pulled, replacing missing teeth was a nightmare, especially when dentures were not the desired solution.
But now, modern implantology makes it possible to firmly anchor artificial roots in the jawbone that support high-quality, functional and esthetic fixed prostheses.
What is a dental implant?
Oral implantology – or dental implantology – makes it possible to reconstitute lost dental elements by inserting an artificial root called an “implant” into the jawbone.
Fixed dental prostheses (crowns, bridges) or removable dental prostheses, commonly called “dentures” (stabilization of complete or partial removable prostheses) can be attached to this artificial root.
What happens if I don't replace the missing teeth?
Losing a tooth or several teeth without replacing them right away has consequences that are sometimes difficult to remedy:
– displacement of neighboring teeth
– progressive loosening of the teeth of the other jaw which do not find teeth to butt against
– disappearance of the bone in which the lost tooth was located, making it more difficult to insert an implant after a few months
Inserting a dental implant immediately prevents the movement of the other teeth, preserves the bone and immediately restores chewing and, with it, a feeling of comfort and security at every moment of the day.
What are the implants made of?
Current implants are made of a titanium alloy (some are made of zirconium oxide) which guarantees their perfect acceptance by the body (biocompatibility).
Is rejection possible?
No, the implants are 100% biocompatible.
Are there cases of implant failure?
The probability of failure is extremely low and only in about 3% of the cases, the osseointegration (union) between the implant and the bone may not work. If this happens, the implant simply needs to be replaced and this replacement is done free of charge (only the live costs will be charged to you).
Another cause of failure can be post-operative infection, but thanks to antibiotics, the incidence is also very low. The cost of “repair” in case of implant loss due to infection is the cost of the treatment.
It should be noted that smoking increases the risk of failure, as does poor hygiene or failure to follow instructions; in these cases, the re-intervention is completely at your expense.
Should I expect to have pain after the implant procedure? How long does the sick leave last?
During the operation and immediately afterwards, you have nothing to fear because the medication administered by your surgeon suppresses the pain.
Later on, any pain that may occur is well bearable and easy to control thanks to the painkillers that we provide you with.
During the first few days, swelling may occur and be visible. However, in most cases, the discomfort disappears completely after 5 to 7 days.
You will normally be able to return to work the day after the procedure, but a work stoppage is nevertheless established for two or more days, depending on the individual reaction or special requirements.
In the case of a flapless procedure, you will be extremely comfortable because there is no swelling, no bleeding, no pain… it’s magic!
F.A.Q. All-on-4 treatment
How many appointments are required to perform a restoration with a FIXED full denture (12 teeth)?
The number of treatment sessions is significantly reduced compared to traditional methods: with the All-On-4 technique, only one surgical procedure is required.
As a rule, 3 to 4 appointments are required for planning. Afterwards, the implant placement and the temporary prosthesis are done in one session. A few months later, 3 or 4 appointments are needed to finalize the bridge.
Here is a summary of the treatment steps:
1. Consultation: patient’s request or doctor’s proposal. Communication of the basic principles of the treatment and the approximate cost
2. Optical impressions, photos
3. Second optical impression and 3D radiography
4. Discussion of details, choice of prosthetic solution, treatment plan and final estimate.
5. Intervention and provisional prosthesis with immediate loading
6. Checking and removal of sutures
7. Control
8. Final impression
9. Aesthetic test
10. Placement of the final prosthesis
11. Final check-up
Other appointments may be necessary for technical reasons or to provide more information at the patient’s request.
How do I clean an All-On-4 prosthesis?
It is important to clean around the implants and under the prosthesis (the part in contact with the gum).
The prosthesis is designed to be cleaned well underneath with dental floss or special brushes.
Occasionally, the special anatomy of a patient may require some practice to ensure a successful cleaning. The office staff will give you all the details to help you get the best result.
What to do when the prosthesis is damaged?
In general, the All-On-4 implant-supported bridge consists of a metal bar created by a digital system and then covered with resin or composite teeth.
This method of construction is aesthetically and functionally perfect and makes it easy to repair. For example, if one end of the All-On-4 bridge breaks, it can be easily repaired: the prosthesis can be unscrewed in a few minutes and we can have it repaired in our laboratory. The screw-retained prosthesis is a great advantage over cement-retained bridges, which are impossible to remove.
If a patient has special requirements, we can make the fixed bridge out of an even more aesthetic material: the zirconia-ceramic bridge. It is important to know that this prosthesis is more aesthetic and more resistant, but in case of a breakage (very rare), the repair is more complicated.
Can I talk to other patients who have had all their teeth replaced before making a decision?
Of course. We have a list of patients who are willing to share their own experiences.
